Baptism for children (infants through age 6):
Congratulations on choosing to baptize your child into our awesome Catholic Church! Our family of God at St. Martin de Porres Church welcomes you with our love and prayers on this journey of faith.

• Baptismal families should be registered members of St Martin de Porres Church or related to members of our church. If registered at another parish, then provide a Letter of Permission to baptize your child here from your own parish. 
• A copy of the child's birth certificate must be submitted.
• Godparents must be fully initiated (Baptism, Eucharist & Confirmation) and practicing Catholics in their registered parish. If married, they need to be sacramentally married in the Catholic Church. Only ONE godparent is required, but if choosing TWO they must be male & female.
• Parents and Godparents need to schedule and attend Baptism Class at St. Martin de Porres or another parish and provide class attendance certificate. If any have attended a Baptism Class within the last 3 years then you need not repeat if you provide a copy of attendance certificate.  Prior to attending the Baptism Class, please watch the "Reborn Session 1- A New Creation" on Formed.org. You can register for free on www.smdpyl.formed.org. To sign-up for a Baptism Class at St. Martin, email [email protected]with the names of those attending.

• Parents need to review the Baptism Guideline Packet, submit the Baptism Enrollment Form, provide Parish Letter of Permission (if not registered at St. Martin de Porres) and Baptism class documents. 
• Godparents need to submit the Godparent Verification Form and Baptism Class documents.
• There is a suggested donation of $100 for group Baptisms. Donations may be submitted to the Parish Office. 
• Once all completed documents are submitted to the parish office, we schedule your Baptism date.  Baptisms are scheduled at 2:00 PM on a Sunday. Upcoming Baptism dates:
